  1. Calcium was adjusted for serum albumin according to the equation: corrected Ca = measured Ca + (4.0 - serum albumin in g/dL)
  2. Urea nitrogen, creatinine, and hematocrit were excluded in crude analysis because of a strong association with normalized PCR, creatinine generation rate, and hemoglobin, respectively
